Co-op Scholarship Deferment Request Form

This form is for current students working through UA’s Cooperative Education Program to defer scholarship awards. Co-op students may not use scholarship awards during work semesters. Requests to reserve use of scholarship awards for future semesters must be submitted using this form. If request is denied, notification will be sent via your Crimson email account.

By submitting this form, I acknowledge that final approval of this request rests with the appropriate UA scholarship officials and that approval is not guaranteed.

PLEASE NOTE: By default, scholarship(s) will be applied to the semester specified under “Next Academic Term.”
